Risk Assessment

Date risk assessment completed: October 2024 – Next due for review October 2025

Organisation name: Strimech Engineering ltd

Machinery Type: BT2000 BOX TIPPER, JCB P/no. 332/V3960

Description: To fit Class 3/ITA3 (Teletruck) type machines

NOTE; ALL Operators must read and understand the OPERATORS MANUAL & RISK ASSESSMENT before using the Box Tipper

The OPERATORS MANUAL & RISK ASSESSMENT must be readily available for reference when required.

What are the Hazzards? Who might be harmed and how? Risk Evaluation levels 1-5 (1= Low Risk 5= High Risk Precautions in Place Further action required / Notes?

Entanglement, trapping, impact by collision and crushing during use.

Bystanders unnoticed by operator during use by being in close proximity or not paying attention to their surroundings.
2
See note in operator manual about beibng aware of wandering individuals particularly children.
Note in operator manual states all operators must have experience and hold appropriate license to use this equipment when attached to the Telehandler.
Injury caused by inadvertently operating the box rotator during maintenance e.g. by another person
Operator/Bystanders
2
safety instructions included in operator manual states the operator must “switch off the telehandler for maintenance and repair work and ensure that it is protected against inadvertent starting by:

– Locking the principal controls and removing the ignition key and/or

– Detaching the hydraulic supply.”

Operators advised to remain vigilant during routine maintenance and only experienced individuals are allowed to preform maintenance on the box tipper.
Injury from high pressure hydraulic system including burns
Operator/Bystander
3
Operator manual stipulates only working on the hydraulic system when the hydraulic system is depressurised and the drive is switched off. Any defective hoses and connections must be replaced and not repaired to maintain safety, Hydraulic oil under pressure is hot, see warning in operator manual.
Strimech is not responsible for damage to produce caused by hydraulic oil leakage or injuries caused by not following the safe working practices regarding pressure systems and only experienced individuals should preform maintenance on the machine.
Crushing injury via box falling off machine
Operator/Bystanders
2
See operator manual for correct and safe adjustment of top/side arm assembly relative to boxes being used.
Safety note in operators manual – Reminder to adjust the side/top arm assembly for different box sizes.
Excessive space between the box and the arms of the box tipper is dangerous and by allowing too much slack/free movement could result in losing the box off the box tipper. Operators advised to keep the Telehandler carriage and box tipper tilted slightly back to keep the load against the Telehandler carriage and prevent the box sliding off
Sized box heights so adjustment is quick and easy to preform on box tipper
Shock loading/overloading causing damage/failure
operator/Bystanders
2
Operator manual states: Please be aware of increased weight due to shock loading. This includes preforming manoeuvres such as driving down a rough field, at some speed, with a full box. The resulting bouncing effect is known as shock loading and the weights can be significantly magnified to the point of overload. Operators are advices to use the box tipper for the use it is intended for -lifting and carrying of free-flowing produce at a time, to prevent overloading
Overloading to the point of exceeding max capacity of machine will cause damage to main pivot pin to the point of sheering the carriage could detach and fall with a full load.

Operators need to be aware of box tippers capacity (on the serial plate attached to machine)
Stab/puncture from unguarded pallet tines
Other road users and traffic
1
Considered low risk, operator manual states that when travelling on public roads the pallet forks and arms on the box tipper should be removed due to their protrusion and potential risk for accident/injury particularly when negotiating junctions or in traffic.

whilst in the field, operators are warned to watch out for wandering individuals, particularly children.
Operator’s with appropriate forklift license should be aware of risks on public highways
injury from working/standing under suspended load when in use
Bystanders
2
General health and safety always stipulates to never work or walk under a suspended load. Risk of crushing if box tipper/box/contents of box falls. Warning noted in operator manual
Operators with the appropriate Telehandler license should know to be aware of individuals being too close to avoid the risk at all times.
Connecting hydraulic pipes incorrectly
Operator/Bystander
1
Operators with the appropriate license have experience in connecting and disconnecting hydraulic pipes
replace damages/defective hydraulic hoses and connections only DO NOT REPAIR THEM
Trapping/crushing whilst connecting/disconnecting Box Tipper to Telehandler
Operators/Bystanders
2
Operators must be experienced and hold appropriate Telehandler license t use. See operator manual about being aware of wandering individuals particularly children. Instruction on safe connection to loader is included in the operator manual.

Risk noted in operator manual: Never allow any person to stand between the Telehandler and the box tipper during connection at any time

notice about not wearing loose fitting clothes to prevent trapping in operator manual.

Operator does not need to leave Telehandler cab to locate a box onto the box tipper as long as the top arm/side arm is assembly are set at the correct position, see operator manual for safe adjustment of the box tipper.
Continue to supply mobile application download information with delivery of box tipper.

PDF version via email upon dispatch and make sure website copies are up to date.
Couplings and if the two pipes are connected the wrong way around, the controls in the forklift cab will work opposite to what is expected which may inadvertently cause injury, The connections will need to be safely disconnected and then reconnected the correct way (remembering to depressurise the hydraulic system first)
Non maintenance and faults causing hazardous operation
Bystanders
2
Non maintenance can result in any machine becoming dangerous. Operator manual states: Regular maintenance is a prerequisite for good operational reliability
Trained personnel only may execute maintenance activities. Delegate a supervisor!

The rotator is a low maintenance unit but still requires regular greasing routine to maintain operational success.
